RESERVATION POLICIES:

Reservations may be made up to two days in advance. Dr. Sara strongly recommends that one parent be present during the first appointment. If this is not feasible, it is OK for a caregiver to be present, subject to completing a waiver. 

You must complete a health history questionnaire and waiver for your child prior to the appointment. All forms and signatures are electronic and will be provided through Dr. Sara's secure, HIPAA-compliant client portal. 

A credit card is required in order to reserve an appointment. 

CANCELLATION POLICY:

We ask that you cancel or reschedule at least 24 hours in advance of your scheduled appointment, or you may be charged the full cost of the appointment. We appreciate your understanding. 

fees:

The standard fee for the home-based dental consult, exam, cleaning, and oral health education is $200.

billing and payment:

Your credit card will be automatically charged after the appointment. While Dr. Sara does not accept insurance at this time, she may do so in the future. She is happy to assist you in obtaining reimbursement from your insurance provider.
