Once you select your date and time, you will be asked to complete standard intake and patient information forms prior to your child's appointment. These will all be made available electronically. 

Currently, Dr. Sara does not accept insurance, however she will assist you in obtaining insurance reimbursements. Please contact Dr. Sara for inquiries regarding insurance reimbursement.
